Welcome to the Ninth Annual Survey of Owners focused on Risk Management. Your participation is invaluable to this research project.

DIRECTIONS:

When you are finished, it is important that you click on Submit at the end of the questionnaire, or your responses will not be saved.

For each risk, you are required to answer three questions: One regarding frequency and two regarding severity.
For each risk, you can optionally enter a third severity type that is not listed.
For each risk category, you can enter a risk type that is not listed.
Use the buttons at the bottom of the page you are on to navigate the survey. Do not use your internet browser's BACK button.
